These phrases are used so much that they constantly show up in writing because they (sometimes) reflect how we normally speak. We try to keep the dialogue “real” but often times it can be too much of a good thing.

True. Dialogue is supposed to LOOK like real speech, but it isn't real speech.I've got phrases and words that show up too often in my narrative as well. I discovered I have to search for 'moment' as I use it far too often.
